Your senior has pre-registered and purchased a ticket to the June 6 2021 Senior Celebration. This consent form must be reviewed and completed the student's parent/guardian by June 3, 2021. If you submit it after June 3, please print a copy of the completed form and bring it to check-in. Thank you!

Consent to rule of conduct at the event. Click on all that apply. I understand that: *

Every student must present his/her current school picture ID at check-in.

All students will be subject to a breathalyzer test at check-in.

Students must check-in for the event at the school no later than 9PM.

The possession or consumption of tobacco, drugs, alcohol and paraphernalia is strictly prohibited. Required prescription must have proper label showing student name or will be confiscated. Depending on behavior, students may be subject to arrest.

No large bags or backpack will be allowed and all bags will be searched at check-in.

No outside food or drinks allowed. No water bottles.

No re-entry for the event.

Students are responsible for their own transportation to/from the event.

All students must depart school grounds by 11:20PM.

Any participant, whether in the school building, the parking lot or in an automobile on school property, will comply with all school rules, regulations and reasonable requests from the Newton South PTSO.

Face masks are optional outdoors. Face mask are required indoors when using the bathroom.

I agree that parent and faculty volunteers of this event have my permission to check my student for any alcoholic beverages or any other illegal substances that may prevent my student from attending the event. If my student is found to have alcohol or other illegal substances, I will be called immediately at the phone listed on this waiver to pick them up immediately..
