By printing your name below, the undersigned does hereby and forever discharge “SPARK Summer Dance Intensive” and “DanceFest Productions, Inc.” and its directors and staff of and from all manner of actions, suits, damages, claims and demands whatsoever in law or equity from any loss or damage to the undersigned's property or personal injury or COVID-19-related illness which may occur while participating in any activity related to this event. Additionally, the undersigned grants permission for “SPARK Summer Dance Intensive” and “DanceFest Productions, Inc.” to use any photos taken during the festival for publicity or on the DanceFest Productions, Inc. website and materials. No names will be used and no compensation will be given. *
