This form is intended for WPC members only. WPC is piloting an equipment loan request program, where club members can request equipment for personal usage.

In filling out this form, WPC members agree to:
1. Optimally use and take care of WPC equipment
2. Hold WPC accountable for missing or damaged equipment and make WPC aware as soon as possible
3. Email williamsphotoclub@gmail.com with concerns or questions as well as notify WPC as soon as possible of retuned equipment'
4. Return equipment within the listed time span

At this time, WPC is offering equipment loans for up to 36 hours. WPC reserves to right to withhold equipment loans if timing conflicts with a WPC event where club equipment is needed and essential.

Be aware that event request forms from organizations, departments, and offices take precedence over equipment loan request forms.
