After you have put in your order for your embroidered or non-embroidered standard prolid diaper/s , you must fill out this form!  It must be filled out within 1-2 hours of purchase.  Fill it out SEPARATELY for EACH custom diaper you purchased.  If you purchased the CUSTOM option, be sure to fill out the CUSTOM FORM.
