Title: Transitions: Coordinating Successful Student Transitions from Middle to High School

Description: The transition from middle school to high school is both an exciting and challenging time for students and parents, particularly for those students who have had unsuccessful school experiences or do not have the support at home that facilitates success. Coordinated efforts between middle schools and high schools that focus specifically on the social and emotional needs of students transitioning to high school will result in a more successful entry into the new school. When both the sending and receiving schools take a perspective that is student-centered, students will be better prepared to be successful in their new school. It is a critical component of the transition process that all students and parents are engaged in their new school in a timely manner. Sample transition plans, including timelines, will be shared from schools that have been successful in meeting the needs of entering students.
