This form is to perform a variety of functions:
• provide important contact information for students joining Briar Woods TSA chapter
• grant/decline consent for photograph/videotape of student
• authorize emergency treatment for student in the event of injury/illness during any TSA event
• Hold harmless TSA, Briar Woods HS and its advisors any and all claims resulting in bodily injury or property
damage while participating in any TSA activity or event
• acknowledge and agree to TSA member conduct
