I hereby release PARKWAY MEDIA PARTNERS LLC dba PHLV RADIO, its agents, employees, INDEPENDENT PRODUCERS and its legal representatives from all claims and liabilities relating to use of said images or audio/video, including but not limited to third party copyright claims.

On my statements or spoken words captured on media, I grant permission to use my statements that were given during an interview, with or without my name, for the purpose of news and public information, advertising and publicity without restriction.
On compensation for the guest appearance, I waive my right to any compensation from PARKWAY MEDIA PARTNERS LLC dba PHLV RADIO, its agents, employees, INDEPENDENT PRODUCERS and its legal representatives.
On guaranteed distribution of my appearance and cost of appearance, I acknowledge that PARKWAY MEDIA PARTNERS LLC dba PHLV RADIO is giving me FREE AIRTIME, FREE PRODUCTION AND ACCOMODATION as a form of voluntary PUBLIC SERVICE and/or a performance of a reciprocal benefit for both parties. However, I understand that PARKWAY MEDIA PARTNERS LLC dba PHLV RADIO does not guarantee the full delivery of said FREE AIRTIME, FREE PRODUCTION AND ACCOMODATION as it may be cancelled without notice or its implementation may be adversely affected by technical difficulties or force majeure. In the event of cancellation or inability to complete guesting appearance, PARKWAY MEDIA PARTNERS LLC dba PHLV RADIO does not guarantee any make-up or replacement schedules. Further, PARKWAY MEDIA PARTNERS LLC dba PHLV RADIO is not obligated to deliver metrics of viewerships/listenerships, insights on social media reach/engagement and other measurement of reach.
On distribution via social media platforms, I understand that portions or entire performances may be unimplemented beyond the control of PHLV Radio. As such, I acknowledge that PHLV Radio cannot guarantee distribution on any third party social media platforms.
On materials performed during my appearance, I acknowledge that I am able to legally perform all materials or portions of any work used without infringing on anyone else’s intellectual property. If any intellectual property claim is made by anyone on any material I perform on PARKWAY MEDIA PARTNERS LLC dba PHLV RADIO, I shall solely answer to the copyright inquiry or infraction if any.
On sponsors or event announcements I mention or promote, I acknowledge that while mention of sponsors or events can be allowed with prior communication with PARKWAY MEDIA PARTNERS LLC dba PHLV RADIO, the station is not in any contract with me to promote or mention any sponsor I carry and the performance of any sponsor deliverable cannot be guaranteed by PARKWAY MEDIA PARTNERS LLC dba PHLV RADIO. PARKWAY MEDIA PARTNERS LLC dba PHLV RADIO reserves the right to censure any broadcast/mention of any unauthorized sponsor. Further, PARKWAY MEDIA PARTNERS LLC dba PHLV RADIO is not obligated to deliver metrics of viewerships/listenerships, insights on social media reach/engagement and other measurement of reach.
I understand that the scheduled interview may be subject to change due to various factors, including technical issues, force majeure, and other unforeseen circumstances. While the station is not obligated to reschedule, both parties are open to discussing the possibility of arranging a new interview time.
By agreeing to appear as a guest on PHLV Radio, I am doing so voluntarily and of my own free will. I understand that I am solely responsible for any risks associated with my physical visit to the studio, and I hereby release the station from any liability or claims that may arise as a result.
