St. Catharines Collegiate Indoor Spectator Attendance

If you wish to be a spectator for a game being played at St. Catharines Collegiate Secondary School, you must complete this is form by 1:30 pm on game day. If you wish to be a spectator for a game being played at another school, you must visit their school website and fill out their Spectator Attendance Form.

Each athlete may have two (2) guests attend - if they are 14 days past their second vaccination (passport shown upon entry), wear a mask indoors at all times, successfully pass the screening with no symptoms and abide by all staff instructions.

Notes:
You must complete this form for each guest.
You can only have 2 guests.
You may not bring any additional guests.
If this form is not completed in advance guests may not be allowed entry into the event.
