GYROTONIC® Waiver - Christine Kardell
The undersigned "Participant" has contracted with CHRISTINE KARDELL to be trained and instructed in an exercise program not limited to GYROTONIC® and/or GYROKINESIS®.  Participant assumes full responsibility for any injuries or damages which may occur in connection with the above exercise program.  Participant fully and forever waives, releases from liability and discharges CHRISTINE KARDELL and GYROTONIC® St Louis Central, LLC from any negligence or any negligent act or omission, from any action or cause of action present or future, whether the same be known or unknown, anticipated or unanticipated, resulting from or arising out of participants use or intended use of the training and/or exercise program.  Participant fully understands the risks involved in respect to such exercise program and training and participant signs this waiver voluntarily.  This waiver shall continue until either cancelled by either party in writing.

COVID-19 Release of Liability
Participant recognizes that Corona Virus or COVID-19, is an extremely contagious virus that spreads easily through airborne and droplet transmission and person-to person contact and maybe transmitted by individuals having no apparent symptoms of infection.  Federal and state authorities recommend social distancing as a means to prevent the spread of the virus. COVID-19 can lead to severe illness, personal injury, permanent disability, and death. Participating in Gyrotonic St Louis Central LLC/Body
Fusion LLC programs or activities or accessing Gyrotonic St Louis Central LLC/Body Fusion LLC facilities could increase the risk of contracting COVID-19. Participant agrees to comply with the rules and guidelines adopted by Gyrotonic St Louis Central LLC/Body Fusion LLC relating to COVID-19 while using Gyrotonic St Louis Central LLC/Body Fusion LLC facilities and participating in Gyrotonic St Louis Central LLC/Body Fusion LLC programs and activities.  Covid - 19 policies can be found on the Gyrotonic St Louis Central website or from studio office.
CANCELLATION POLICY:  Appointments may be cancelled by calling your instructor directly at least 24 hours in advance to avoid being charged for the appointment. *
