By completing this form, you are requesting one (1) hotspot for your household, have at least one student in your household that is in grades 3-12 for the current school year, and have reviewed the "Lakota Local Schools Student Hotspot Connectivity Program Handbook". Upon completing the form, the student designated as the pick up person may retrieve the hotspot from the Library/Media Center during normal hours.
