This form is to be used to capture information to be directed to animal control, including stray/roaming dog reports or situations where pet owners may not be meeting the standards of the law (food, water, shelter, medical care). Encourage residents to contact Animal Control directly at 618-235-0585 for the quickest response, but also capture details using this form that we will forward to AC and follow-up with them as necessary. If this is an emergency type of situation and you are filling out this form, please make sure to contact Animal Control via phone at 618-235-0585 to report the issue as soon as you have submitted the form.
