Authorization
I hereby authorize the Counseling Office / Registrar at Crean Lutheran High School to release my unofficial student transcript to collegiate athletic programs. I understand these programs will have access to information included on the transcript, which contains the following information: name, address, telephone number, birthdate, and academic history. Unless indicated otherwise, this release shall remain in effect until I graduate.
