I understand my membership will cancel 30 days after this Membership Cancellation Request Form is submitted so long as there are no months remaining on my plan's contract terms. Otherwise, the Membership will cancel 30 days after the final monthly payment installment on my contract is billed. I will be billed the final payment during this 30 days. There are no refunds.
