Please complete this form to submit your household’s weekly Meals-To-Go order for VIRTUAL students. The Meals-to-Go Program will provide 5 shelf-stable breakfasts and 5 shelf-stable lunches for the following week.

Please have the order form submitted by 3:00 pm on Thursday, November 19th.

This is for the Friday, November 20th pick-up date.

Gueydan High School / 1:00 pm - 2:30 pm
Forked Island E. Broussard / 9:00 am - 11:00 am

Parents/guardians should submit one order form for their household each week, selecting the location that is most convenient for them. Once the form is submitted, parents will receive a confirmation email that MUST be shown when picking up meals. Parents may choose to have someone else to pick up their meals, but this person must have the email or a screenshot of the email. All pick-ups will be drive-thru only, and drivers should remain in their vehicle at all times.
