Button orders may be submitted AFTER the team's photo proofs are available to view online. Please do not submit an order until you know which photo of your student you want used on the button. Submitting an order without a photo will delay your order. Button photos will appear exactly as in proofs. Please view the proofs and choose your photo before you submit this form. You will need to enter the _bea# of the proof you choose on this form. This Google form must be submitted to generate a button order.
PLEASE COMPLETE ALL INFORMATION EXACTLY AS IT IS REQUESTED. MISSING INFORMATION WILL DELAY RECEIPT OF YOUR BUTTONS. If buttons are ordered and payment is received by the deadlines posted below, you will receive your buttons within 10-14 days.. If buttons are ordered and/or payment is received after the deadlines listed below, buttons will be delivered as quickly as possible, but no delivery time is guaranteed. Buttons will not be made until payment is received.
Once the form is submitted, the payment link will display on the order confirmation screen. Buttons are $10 each. Orders and payments are completely separate. You do not need to submit your order again if you have trouble with the MBHS payment system.
NOTE: Payment link will be displayed after you submit the google form. If you cannot get the payment link to work, you can submit a check to the high school, attention Heather Fitch, payable to MBHS Dorians. Please note player's name and "Brag Button" on the check.
