If your child will turn five years old prior to August 1, 2024 and you are planning to enroll him/her as a kindergarten student in The Clever R-V School District for the 2024-25 school year, please submit the following form. This information will also be used to contact you when we begin scheduling for kindergarten screening in late spring 2024. By submitting this form, you are taking the first step in registering your child for kindergarten at Clever R-V. Please note that the next step in the registration process is to submit the following documents: birth certificate, proof of residency, and immunization records. Documents may be presented in person to the Clever PK-2 office, by fax (417-743-4812) or by emailing Taffy Keltner at keltnert@cleverbluejays.org. Please do so at your earliest possible convenience. If your child is already enrolled in our preschool program, we already have your information on file, therefore, you will only need to submit updated shot records. *
