Please answer the following questions in a separate form for each of your children, grades K-6th. This is for second trimester, which runs November 16th through February 26th. Surveys must be returned by Friday, October 2nd. Data will be used to plan schedules and allocate personnel for second trimester. It is IMPERATIVE that you return the form to allow us to plan accordingly. Thank you!!
