I/We also agree that any outstanding veterinarian bills related to the animal(s) listed herein, prior to surrender, is the sole responsibility of the above surrendering owner(s). I/We certify and understand that by signing this surrender form, I/we relinquish any rights to the animal(s) listed herein and the animal(s) will not be available to be returned. *
I/We certify that if we have made special arrangements with The Hope Highway and plan to take our animal(s) back, all medical expenses incurred while in The Hope Highway’s care will need to be reimbursed to The Hope Highway.
