Did you forget to send in an excuse for your child's absence?  Please fill out the excuse below and submit to the school office.  If we do not receive an excuse or this form returned with in 2 days, your child's attendance will be listed as an unexcused absence.  If you have a doctors excuse,  please send that in as well. Thank you!

Please note:  Each student can have up to 10 applicable absences (parent written excuse) per school year.  A doctor excuse is required once a student has reached their limit.  Also please remember that if a student has received an unexcused absence, they can receive 0% on all missed work for that day.
