Furze Platt Junior School is registered as a data controller to process data. Any information you provide will be treated in the strictest confidence and will only be used for School purposes.

Furze Plat Junior School confirms that it shall only use photographic/video images of young people in line with the information Commissioner's Office Code of Practice to demonstrate or promote activities & events relating to school & curricula provision. A copy of the Code of Practice can be obtained from ICO - www.ice.gov.uk parents should complete the form for those children's aged 13 years and under.

Please think carefully when completing this form. We do use images of pupils for our newsletter, website and SeeSaw accounts. Group photos are taken on school visits / sports events which can be used for these accounts plus local press (i.e. Maidenhead Advertiser). If you indicate 'No' to 'Use of Image' your child will be excluded from any photographs that are taken. If you do not give any indication with regard to 'use of name', we will assume you are NOT happy for your child to be named
