GPS is offering COVID-testing kits to be utilized upon our return from April break. Please complete this form, indicating whether or not you would like a kit to go home with your student next week prior to Spring Break.  We will please need a response to this form by Monday at 4 PM to ensure we have enough kits for all who request one.

NOTE: If you have multiple student or siblings within the same school, you only need to fill this out once. Kits come with two tests each.

GPS ofrece kits de prueba de COVID que se utilizarán a nuestro regreso de las vacaciones de abril. Complete este formulario, indicando si desea o no que un kit se lleve a casa con su estudiante la próxima semana antes de las vacaciones de primavera. Necesitaremos una respuesta a este formulario antes del lunes a las 4:00pm para garantizar que tengamos suficientes kits para todos los que soliciten uno.

NOTA: Si tiene varios estudiantes o hermanos dentro de la misma escuela, solo necesita completar esto una vez. Los kits vienen con dos pruebas cada uno.
