MHPCS Uniform Order Form SY 22-23
Please fill out this form if you need to order uniforms for your keiki. We have a limited amount of inventory so may or may not be able to fill the entire order. If you have more than one keiki please fill out 1 form per keiki.

Uniform Policy for MHPCS is Monday-Thursday Cotton Shirts, Friday is free dress
Dri-fit shirts are for huakai days only

Prices are as followed
Cotton-YS-Adult Xl $9, 2xl $11, 3xl-$13
dri-fit-YS-Adult XL $14 , 2xl $16, 3xl $18
Middle School Polo YM-YL $19, AS-AXL-$21, AXXL-$23,  AXXL-$25

 You can either pay in cash/check to office or pay with your credit card online. A 3.55% transaction fee will be added to the total of all credit card payments and Invoices will be emailed.
A copy of your responses will be emailed to the address you provided.
