Tutor Absence Form
Complete this form any time you will be/are absent from your shift. Remember that this form must be completed BEFORE your scheduled appointment time, or it will count as an Unexcused absence.

DROP-IN TUTORS:
For Oasis Appts: PLEASE DELETE YOUR AVAILABILITY AND CANCEL YOUR APPOINTMENTS ON OASIS!
For Zoom Drop-In: Email the front desk at aatc-frontdesk@ucdavis.edu

INDIVIDUAL TUTORS: Please be sure to notify your tutee(s) that you will not make your scheduled appointment(s). Their email should be included in the Individual Tutoring assignment email you received.

(NOT APPLICABLE SPRING QUARTER) RESIDENCE HALL TUTORS: Please be sure to notify the Residence Halls of your absence by emailing aactutor@ucdavis.edu. If it is after 4pm, please ALSO call (530) 752-5566. In both cases, let them know the reason for your absence and which subject you would have tutored.

If you are missing multiple shifts in a day, list them out on one line, separated by a comma.
