This form is to cancel a campus housing assignment for an upcoming semester. If a student has checked into a residence and picked up a key, a request for an early release must be also be submitted and approval received before an assignment may be cancelled. The form to request for an early release may be downloaded from the Housing website at www.housing.eku.edu.

I have lived 4 semesters on campus.

I am transferring to another institution.

I have withdrawn from the university and have cancelled my class schedule.

I will be taking all my classes at an EKU regional campus (listed below).

I have received approval from a release from my contract.

I do NOT meet any of the conditions described above, but I am going to live at home with my parents in their primary residence that is within 50 miles of the Richmond campus. I understand that I must complete and submit a Verification of Living with Parent's Form before housing charges will be removed from my account. (This option is only available if you have not checked into a residence hall.)
