Honorary Service Awards - Brookhaven PTA 2023-2024
Do you know a Brookhaven staff member, teacher, parent, and/or volunteer who has gone above and beyond to support our school and community?
The PTA would like you to let us know! And we understand it has been a few years of little to no volunteering, so it doesn’t have to be from this year specifically. Those nominated will be considered to receive an Honorary Service Award.
The Honorary Service Awards will be presented on April 27, 2023 at 5:45PM at our General Association PTA meeting before Open House.
Please complete and submit this form by Tuesday, April 18, 2023 for your nominees consideration.
To nominate someone, choose the category below you feel is most fitting for the individual you would like to nominate, write their first and last name and then add a description of their contribution to our school (include any details that you feel describe their positive impact on our school). You can nominate one person or multiple people using this same form. If you have more than one person for a specific award, please use a second form.
