Three Springs Health and Wellbeing Survey
The new Public Health Act 2016 requires all local governments to develop a local Public Health Plan.  This survey is being run to help inform the Shire of Three Springs first Public Health Plan.  The Plan will prioritise actions that seek to support healthier lifestyle opportunities, environments and outcomes for our residents with the aim to maximise community health and wellbeing.

Your input is vital and will assist in shaping this plan, we encourage you to take 10 minutes to complete the survey.  The information you provide will be kept confidential in accordance with the Privacy Act 1998.

Please ensure that you have completed your survey before 31st August 2020.
