2022 CGAA Baseball COVID Reporting Form
Parents/guardians are required to report any positive COVID case for their child who is a participant in CGAA Baseball (Traveling or In-House) and follow the then current CDC quarantine and isolation guidelines at: https://www.cdc.gov/coronavirus/2019-ncov/your-health/quarantine-isolation.html.

The information below will be submitted to the CGAA Baseball President, acting as the CGAA Baseball COVID Compliance Manager. This information will be used to assist in the length of any required individual or team quarantine. For reported positive cases, the athlete's name will be shared with CGAA Baseball and the head coach. Parents are responsible for reporting all cases to the athlete's school.
