This form is considered PART 2 of the SAA Mentee Application and should ONLY be completed by the Personal/Coach Reference named/identified by the Mentee Applicant. The Mentee should NOT complete this portion herself.

Applications will be accepted until September 1 of each year for consideration of support in the next calendar year. (Incomplete and/or late applications will be considered early submission for future calendar)

The primary criteria used to select an applicant includes, playing ability, age, financial need and personal recommendations.  Application and Personal References must be submitted on on before September 1 (No Exceptions)

ELIGIBILITY GUIDELINES

Be a minimum age of 18 years.
Be able to consistently shoot no more than 3 over par on a minimum  6200-yard course, par or better preferred.

APPLICATION REQUIREMENTS
Electronic Application submitted on or before September 1
Two (2) personal reference forms submitted electronically on or before September 1  (One of which shall be from your golf coach/instructor)
Incomplete and/or late applications will  be considered for subsequent years if not received by deadline.
Recipients must reapply annually for continued support.

WHAT HAPPENS NEXT?
Applicant will be notified in writing, regarding final selection.
Monetary awards will be made payable directly to the provider of goods/services and paid to same for the benefit of the recipient, as required.
