This form is provided for your convenience should you need to make a change in your enrollment. For Kindermusik and Group Lessons, pause and withdrawal notices must be submitted before the 15th of the month to avoid charges for the next month. For Private Lessons, pause and withdrawal notices must be submitted by the 1st of the month to avoid charges for the next month.

*Because of the unexpected transition to online class experiences, due to the Corona Virus, we are being more lenient with our withdrawal notice policy. Please understand that if you withdraw close to the end of the month, we will do our best to adjust your payment before the 1st, if not, please reach out to use and we will do our best to make it right.
