CCSD Facilities Request Form
All organizations requesting to use a CCSD facility should complete this form 30 days prior to the event. All cancellations or rescheduling of the event must be presented in writing to Niki Huff at least 10 calendar days in advance of the scheduled event date. Failure to cancel an event in the designated time frame will result in appropriate fees being charged for CCSD staff providing services for the event. It's crucial to adhere to CCSD

When renting CCSD facilities, any food trucks on the premises must provide the below documentation, this is required for all schools/facilities in the district
1) Certificate of Insurance
2) Business License/Service Permit
3) Food Service Inspection Report
4) Completed Immigration Form
