Denison High School Band: Fall Absence Request Form
Please fill out the following form if you know that you are going to be absent and miss a band rehearsal. Mrs. Lewis must also be notified via email if you are going to miss a rehearsal (mhlewis@denisonisd.net).

In addition, please be aware that filling out this request form does NOT mean that your absence will be approved and/or excused. According to the Denison High School Band Handbook, the following reasons may be considered for approved absences:
- Death in the family
- Illness
- Emergency medial procedures

Mrs. Lewis will have discretion on if an absence is approved.
