Welcome to Pre-Optometry Society at UCLA! 

Please fill out this form to complete the membership application for the 2024-2025 term! In addition to filling out this form, please submit the annual $15 membership fee (this covers all 3 quarters!) to Chon Nhu Nguyen, our Finance Coordinator, @chonnhunguyen on Venmo. Please write "Pre-Opt Club fee + Your Full Name" in the Venmo payment. If you don't have a Venmo, please use PayPal to send $15 to uclapreopt@gmail.com, or you can pay us in cash in person at our general meetings! Thank you!

1.) Email notifications of volunteer, shadowing, and work opportunities 
2.) Eligibility for Pre-Optometry Society Scholarships 
3.) Participation in Mentorship Program (receive an undergrad and post-graduate (optometrist/ optometry student) mentor) and Family System
4.) GM Recap emails with minutes and Powerpoint slides
5.) Free attendance at the annual Optometric Symposium
6.) Free graduation cords 
7.) Access to our test bank (includes exams and optometry resources)

Active Member Requirements:
*In order to receive your free graduation cord, apply for Pre-Optometry Society Scholarships, and receive recognition awards, you must be considered an ACTIVE Member by fulfilling these requirements:
1.) Submit this form along with the annual $15 fee (via Venmo to @chonnhunguyen) 
2.) Attend at least 2 meetings per quarter or a TOTAL of 6 meetings for the year
3.) Attend at least 1 social event every year
4.) Attend at least 1 fundraising event a year

For those that join later in the quarter, please note that we will add you to a family system the following quarter. Mentorship is also available at the beginning of each quarter, but you must fill out the mentorship form https://forms.gle/jgLt6yWGKZdw2rVs8!
