Sprout Therapy offers summer camps that are specialized for students with special needs and/or needing special assistance to increase weakness in skills. Some camps may include peer models. Our summer camps maintain a low teacher to student ratio in order to maintain the integrity of our specialization, so spaces are limited and defined in each group. Consultations are held by phone after registration to assist in planning and preparation for each group and to ensure that appropriate support is provided in each group.

PICKUP & DROPOFF:
• Camp is held at Beyond Words, 6054 Royalton Road, North Royalton Ohio 44133
• Parents/Guardians must provide transportation to and from Camp.
• Participants must be accompanied in the parking lot.

COVID-19 (as of March 2021)
• Therapist and Team members will abide by state mandates to protect the participants of each group
• Room & materials will be santized between use of each group. Individual materials will be provided to each participant as to reduce the spread & sharing of materials when applicable
• Waiting rooms and Hallways are currently closed to the public and will not be a place for congregation
• Handwashing & santization will be practiced at the start and end of each group session.
• Temperatures will be taken prior to the start of each session, we ask that anyone with a fever or sign/symptom of illness call to cancel prior to the start of the session.
• If quarantine/illness occurs, arrangements may be made so the child can participate in the group session via virtual participation or may reschedule to a later date if available.

TUITION AND REGISTRATION FEES:
• You are registered for our summer camp with the completion of this online registration form.
• Camp tuition is due in full no later than May 1, 2021. Cash or check made payable to: Sprout Therapy, LLC.. Use of third party funds requires approval before June 1st.
• All fees are non-negotiable

DISCIPLINE & GUIDANCE POLICY:
• Because we have a responsibility to insure the safety of and provide positive experiences for all students in the program, each student is expected to abide by the rules of this program as well as respect the rights of other students. Students will be removed if the safety of other students is compromised.
• Our goal is that each child develops self-control and the ability to work and play with others.
• All students are encouraged to cooperate with staff and other students, as well as, respect self, others and property.

PHOTOGRAPH/VIDEO RELEASE: I hereby consent to Sprout Therapy to reproduce photographs or videos of my child for publicity or advertising purposes. I understand that my child’s name will be kept confidential by the therapists at Sprout Therapy, LLC. and that the Information will only in accordance with ethical standards of professional confidentiality for board certified speech/occupational therapy professionals. I understand that I will not receive financial compensation for the use of this Information.

WAIVER AND RELEASE OF CLAIMS: It is expressly agreed that all use of the equipment and services, and participation in, or a spectator to, any programs conducted within or on the property of the Beyond Words/Sprout Therapy, LLC. shall be undertaken by me, or my child, or my legal ward at my/his/her sole risk. Sprout Therapy, LLC. shall not be liable for injuries or any damages to me, or my child, or my legal ward, or to any of my property, or my child’s property, or my legal ward’s property, or to be subject to any claim, demand, injury or damages whatsoever, including, without any limitation, those injuries and/or damages resulting from acts of active or passive negligence on the part of Sprout Therapy. I, for myself and on behalf of my children, my executors, administrators, legal wards, heirs, assigns and successors, do hereby expressly forever release discharge Sprout Therapy, LLC. from all such claims, demands, injuries, damages, actions or cause of actions whatsoever.

The undersigned further expressly agree(s) that the foregoing Waiver & Release of Claims is intended to be as broad and inclusive as permitted by the laws of the State of Ohio, shall survive the observation, use or participation of the facilities, programs vehicles and equipment by the child/person attending, and that if any provision of this release form, or portion thereof, is held invalid or unenforceable, it is agreed that the balance shall continue in full force and effect.
