SEA Kid Pack Weekend Program
Optional enrollment into weekend Kid Pack program at SEA. Please fill out one form per student.

By filling out this form I further release SEA Elementary, Calvary Lutheran Church, and Food Group and the original donor and hold them free and harmless against all and any liabilities, damages, losses, claims, causes of action and suits of law or inequity or any obligation whatsoever arising out of attributes to any action of any personnel in connection with the collection, storage, distribution, consumption and use or other disposition of the donated and purchased food.

I understand that I am enrolling my child into the optional Weekend KidPack program. On the last day of the week, students will be given a Weekend KidPack, which will be inserted in the child’s existing backpack, with a number of food items inside. This program is completely voluntary and is at no cost to the students and their families. (1st delivery date is Friday, Oct.7)
