One Virus, Many Consequences:   A Call to Action to Stand Together and Build on Evidence to Inform Nutrition Responses

We, the undersigned, as nutrition and food system professionals concerned about the negative effects of the COVID-19 pandemic on the nutritional status of hundreds of millions of children and adults worldwide, have joined forces to inform and support response efforts that will protect and improve diet, nutrition and health outcomes now and post the pandemic.

The COVID-19 pandemic and the shocks caused by containment strategies to reduce transmission could have unprecedented impacts on global hunger and malnutrition and thereby on mortality, human capital and future productivity of nations.

In many countries, the crisis is preying on people whose immune systems are already impaired by malnutrition in all its forms, including micronutrient malnutrition, overweight and diet- related disease. The physical distancing and lockdowns needed to curb the spread of the disease are reducing incomes for families, businesses and governments, disrupting ongoing health, nutrition and social protection programs, and supplies of nutritious foods in an inequitable way.

Unchecked, the crisis threatens people’s access to healthy diets and will increase wasting, stunting, and micronutrient malnutrition, as well as overweight, obesity and diet-related disease. This will lead to increased maternal- and under 5 mortality, increased overall morbidity, poorer learning outcomes and significant livelihood and economic losses, as well as widespread suffering, fundamentally threatening the progress made in maternal and child nutrition and survival over the last few years.

Malnutrition in all its forms may make people more vulnerable to the virus, while the virus and efforts to contain the virus will increase malnutrition and death rates, further increasing vulnerability in a vicious cycle.

CALL TO ACTION:
To better respond to this crisis the global nutrition, health, and food systems communities will need to work together like never before, to assess the scale and reach of the nutritional challenges and to articulate effective responses. Efforts to respond to the crisis need to be based, as much as possible, on good quality data and evidence. While an absence of data is no excuse for inaction, there is also no excuse for ignoring good data and evidence when available.

New challenges require coming together in new ways and with new urgency.

Realising that the nutrition, health and food systems data, models and teams are not as integrated as they could be, and Inspired by the unprecedented collaboration of researchers working on vaccines and therapeutics we have decided to come together to do the following:

Connect relevant systems –economic, food and health— through modelling to get rapid and credible estimates of the overall nutrition impacts of the crisis –at different points in time, for different geographies and populations and on a range of diet and nutritional status outcomes

Learn from previous economic, humanitarian, climate-related crises and pandemics to understand whose nutritional status and which nutrition outcomes are most affected by shocks, and what works to mitigate these impacts.

Based on the above, provide consensus advice on multisectoral actions in the immediate, short and medium run to track, protect, promote and support optimal nutrition. The aim of the advice is not just to rebuild the multi-sectoral intervention infrastructure we had pre-COVID-19, but to adapt and improve it significantly as we move forward to meeting the 2025 World Health Assembly targets and 2030 Sustainable Development Goals.

In the immediate term our work is designed to inform calls to action from nutrition champions at the community, national and international levels so they can catalyse increased commitment to nutrition action. In the near term we will produce a scientific paper summarising the impacts of the crisis on nutrition together with the best evidence based advice we can find on how to address those impacts as the crisis plays out. And from this moment on we will strive to nurture the connections between data, models and teams that we have begun to develop so that we can better stand together for nutrition in response to the crisis -- and beyond.

Our aim is to be as inclusive as possible. Our goal is to bring together as many nutrition, health and food systems researchers from around the world as we can. We operate together through a working group at the heart of the effort and a global group contributing perspectives, ideas and data. All of us agree to use our collective voice to champion the outcomes of the work.

The nutrition consequences of the COVID-19 pandemic, and the range of responses to it, will affect all people, but not equally. Some sections of the population are especially at risk of paying a nutrition penalty and we need to identify and address vulnerabilities to improve resilience. Let us, the global nutrition, health and food systems communities, stand together for everyone, to help track, protect, promote and support nutrition, now, and as we move forward, undeterred from our mission and more determined than ever to end all forms of malnutrition by 2030.
