Child Care Application                                                   Boys and Girls Club &                                                        Mohave Valley Elementary School District
The Boys and Girls Club and MVESD are partnering to support families during Distance Learning.  MVESD starts school on August 5th and continues Distance Learning through the first quarter. We are reaching out to our families that are in need of a program that supports your child's Distance Learning experience throughout the work week.  The Boys and Girls Club will be open for full-day programming 7:30am to 6:00pm with lunch provided, Monday through Friday.  This program is keenly aware of CDC guidelines and will follow social distancing, routine sanitizing, and require face coverings.

Please read through this form, in its entirety, to determine whether your family will need this child-care option in light of school buildings being closed.

Distance Learning
The Boys and Girls Club has an amazing team of mentors who will provide an active, positive place for children.  They will provide classroom monitors to assist students with Distance Learning and to ensure a safe setting to support your children during this time.

Technology
Tablets, laptops and Chromebooks will be used during 'Power Hours' and will be kept in a designated area. All technology MUST be labeled with the child's first and last name. The Boys and Girls Club will not be responsible for theft, breakage or misuse of technology items.

Expectations
Children ages six and older will be required to wear a mask. If your child cannot wear a mask, please identify your extenuating circumstances below.

Fees
The Boys and Girls Club will be charging $40 per week; however,  the partnership with MVESD provides for no-cost supplement scholarships.

By submitting this form, you are identifying that your child is in need of Monday through Friday supervision.  We will be accepting students based on a variety of factors: Essential Workers, At-Risk Children, and children who do not have a safe, supervised place for Distance Learning.

You will be notified by email when your child has been accepted into the program based on the aforementioned criteria.

More information will follow as we learn more about participation numbers from our respective MVESD schools.  Please continue to check MVESD's social media and website for up-to-date information concerning our Road Map for Re-Entry Plan.

It will be crucial for you to communicate with us once we reach out to you as we will be preparing the logistics to begin child care services starting on Monday, August 17, 2020.

** Parents must provide transportation to and from the club.
** Sites have not yet been determined.
** Please contact the Boys and Girls Club if you should have unique circumstances concerning the current programming option - (928) 577-2045.
