Request for Mass Intention
Please use this form to request a Mass for a deceased loved one or for a special intention. Please only one person/couple/family per form/Mass (e.g. Joe Smith or Joe and Jane Smith or The Smith Family). Submit a new form for additional intentions. 

Mass Intentions are published in the bulletin, depending receipt of request and publication deadlines. This form is only monitored during business hours, Monday-Friday. Please allow 24-36 hours for the request to be processed.

If there is a problem we will contact you to choose an alternate date/time.  Due to publication deadlines, the intention may or may  not appear in the bulletin.  Holidays, UF breaks, and emergencies affect our Mass schedules and bulletin publication deadlines.

Suggested Donation and Mass Intention Cards
If you wish to make a donation for the Mass intention, please either use this link or place a check in an envelope with "Mass Intention" clearly marked on the outside and drop it by the reception desk/parish office or in the collection basket on Sunday. Suggested donation is $10.

Mass intention cards are available from the Receptionist or Parish Secretary if you wish to send one to the person/family to let them know a Mass will be/has been said for their intention/loved one. Please stop by during reception desk/office hours to pick out a card!
