** Please note: Health Clinic services and medications are not available for summer camp.  Instructors are not trained to administer aid or medications. In the event of an emergency, they have been instructed to call 911. You are welcome to bring medication to administer to your own student.  

**IF CONTACT CANNOT BE MADE WITH THE ABOVE, THE INSTRUCTOR WILL USE HIS/HER BEST JUDGEMENT TO PROTECT AND ASSIST IN ACCORDANCE WITH THE FOLLOWING POLICIES: A. Caring for the student. B. Notifying the student’s parents or guardian. C. In extreme cases, getting the student under professional care with, or without, family permission.

AUTHORIZATION AND CONSENT TO TREAT A MINOR
In the event my child requires emergency medical care while participating in Rising Star Performing Arts Academ After School Class, I understand that personnel will attempt to contact parents, and if needed will call 911 for emergency medical care and ambulance transportation to the nearest hospital as determined by the responding emergency personnel.

 I understand that neither Community in Christ nor Rising Star Performing Arts Academy, nor its employees, will be responsible for any expenses incurred.

This authorization will remain effective while the above minor is in route, to or from, or involved or participating in the event listed above, unless revoked in writing by the undersigned and delivered to the aforementioned agent.


In consideration of my child’s voluntary participation in the activity listed above, I hereby release and discharge Rising Star Perfroming Arts Academy employees, and Community in Christ for any claim or cause of action, rights, damages and demands of any kind or nature, known or unknown, including claims for attorney’s fees and costs arising out of the aforementioned activity in which I and my child have elected to voluntarily participate.

Your name on this form gives the above student the privilege of joining Rising Star Perfroming Arts Academy classes for which they are registered. I have read the  Participation Agreement  and Health Information and shared them with my student.
