2023 Confirmation Application

In this application, you will find all of your requirements for the 2022-2023 Confirmation year. We ask teenagers submit this application with their parents for two reasons: (1) so that you understand and agree to what is required for Confirmation and (2) for our records. This form is due by 6/1/2022.

In answering the following questions, we ask:

1) That you accurately fill out the record-keeping questions.
2) That you answer the sixth question with your full name (ie: Kathryn Srsen).
3) That you mark "yes" for each following question, acknowledging that you understand that this is a required step to be confirmed.

Confirmation takes place on Friday, May 5th at 7:00pm at St. Albert the Great.

I promise to commit myself to cooperating fully with my parents, my sponsor, and my catechists/teachers as they guide me through the process of becoming a fully initiated member in full communion with the Roman Catholic Church. I understand that I need to complete 2 hours of Justice Service and submit the Reflection Google Form within ONE week of completing these hours. *
