Each student should read and initial this electronic Release and Waiver of Liability, Assumption of Risk and Indemnity Agreement. The Undersigned via electronic signature:

hereby agrees to defend indemnify and save and hold harmless the facility where taiji classes are held and the taiji instructor who includes Charlie Liu and any other instructor, for any loss, liability, damage, or cost which may incur due to the presence of the Undersigned in or upon the premises or by the instructor in any way working for, volunteering, or participating in any event or activity whether caused by the negligence of the venue or instructor or otherwise,

hereby assumes full responsibility for and risk of bodily injury, death, or property damage arising out of or related to the service(s) or event(s), whether caused by the negligence of the venue or instructor or otherwise, and

hereby releases, waives, discharges and covenants not to sue the venue or instructor from all liability to the Undersigned, his/her personal representatives, assigns, heirs, and next of kin for any and all loss or damage, and any claim or demands therefor on account of injury to the person or property or resulting in death of the undersigned arising out of or related to the service(s) or event(s), whether caused by the negligence of the venue or instructor or otherwise.

The undersigned has read this release and waiver of liability, assumption of risk and indemnity agreement, fully understand its terms, understand  that they have given up substantial rights by signing it, and have freely and voluntarily without any inducement, assurance or guarantee  being made to them and intend their signature to be complete and unconditional release of all liability to the greatest extent allowed by law.
