Card Application Form
The cards are for anyone, diagnosis or not. We send these to anyone who is struggling and don't prioritise based on diagnosis', this form is purely to keep details in one secure place. No matter what your struggles are and how you feel they are in comparison to others, we care about everyone equally who applies.

I do not store any information (including addresses) given on this form and the answers are deleted immediately after I write the address on the envelope.

This form doesn't determine who gets a card and who doesn't. It is just so I know who and where to send the card and so I can personalise it a bit.
