Registration is not considered complete until:

you complete and submit this registration form
your OAS (Ontario Artistic Swimming) and CAS (Canadian Artistic Swimming) membership has been processed (if applicable, for new swimmers only)
a copy of your OAS/CAS membership is submitted to treasurer@markhamartisticswimmingclub.com
your payment has been received

Program fees are payable by e-transfer to treasurer@markhamartisticswimmingclub.com
OAS/CAS membership fees are payable by credit card.
