Continuous Enrollment Withdrawal Form for Academy Students

This form is to be used to withdraw your Academy student/s from Continuous Enrollment at United Faith Christian Academy.

Please understand this form extends to withdrawal from either the current year of 2023-2024 or for the upcoming year of 2024-2025.

If you have a preschool student that you wish to withdraw, please contact Christine LaFrancois at Christine.LaFrancois@ufca.org.

If your student/s are graduating in May 2024 or are returning for the 2024-2025 school year PLEASE DO NOT complete this form!

If you do not wish to be charged the $400 continuous enrollment fee, which will be added to your SMART Tuition account on March 1, 2024, please:

·      Read the withdrawal policy in its entirety and

·      Complete and submit this form no later than February 15, 2024. (No exceptions)

 

*Please Note: even if you have paid your tuition in full, the $400 enrollment fee is still due and considered owed, if you do not opt-out of continuous enrollment by February 15th, 2024.

*Please Note: Our Withdrawal Policy: Families withdrawing any time after they’ve been reenrolled are responsible for a $1000 (per student) withdrawal fee.  

*Please Note: Tuition payments for the upcoming academic school year will begin June 1, 2024. 

*Please Note: After July 15, 2024 families withdrawing are also responsible for 50% of the tuition for the remainder of the academic year.

With the above understanding if your child has not been unenrolled prior to June 1, 2024 then you are responsible for the previously agreed upon tuition payment schedule beginning June 1, 2024.

*PLEASE NOTE: PAYMENTS MADE AND ANY FEES PAID ARE NON-REFUNDABLE*.

Withdrawal fees mentioned above,  plus any outstanding monies owed to the school must be paid prior to your student/s being considered officially withdrawn from United Faith Christian Academy. Adrienne Addison, our Business Manager will ensure you receive the correct amount owed.

*PLEASE NOTE THAT IF YOUR ACCOUNT IS NOT SATISFIED, WE CANNOT RELEASE TRANSCRIPTS.*

After all fees that may be due have been paid, and the business office has indicated to enrollment nothing is outstanding then your student/s official notice of withdrawal can be accepted from the date on the form received by United Faith Christian Academy.
