By executing this agreement as a participant in the 2021 Fertile Ground Festival of New Works (the "Festival"), I,
theundersigned, for myself and my successors, heirs, and assigns, agree as follows:

*  My participation in the Festival shall be at my own risk.
*  I will abide by all appropriate rules with regard to participation by members of stage and Actor's Equity unions.
*  I authorize the Fertile Ground Festival of New Works and Portland Area Theatre Alliance (PATA) and their
employees, agents,licensees, successors, and assigns to display, reproduce, perform, distribute, modify, and use
(and to authorize others to do the same), on a perpetual basis, in any media throughout the world, without
compensation my image, likeness, voice, or statements (my "Likeness") as embodied in any pictures, videos, or
other recordings or transcripts submitted to the Festival, in connection with the Festival's business, including without limitation for marketing, promotional, operational, and fundraising purposes.
*  I hereby forever release, waive, and discharge the Fertile Ground Festival of New Works, PATA, and the individual officers,directors, employees, agents and members of either organization, together with their successors and assigns (the "ReleasedParties") from any and all liabilities, claims, demands, actions, expenses (including attorney fees), damages, judgements, andcauses of action whatsoever, known or unknown (together, "Claims") arising from the fault or negligence of the Released Parties,or from use of my Likeness as described above, including without limitation, Claims for bodily injury, death, and property damage, and any other lossof any kind arising out of my participation in the Festival.
*  I will indemnify and defend the Released Parties from and against any Claims asserted by any third party arising from or relating to my participation in the Festival.


If the Festival participant is under 18 years old, I represent and warrant that I am the parent or legal guardian of the participant. I have read and understand the provisions of this document; I consent to the participant taking part in the Festival; and I fully enter into and agree, individually and on behalf of the participant, to the terms of this document.
