First-Time Mentor Application 2023-2024
We are thrilled that you are applying to be a first-time mentor to a member of the CHS Class of 2027! New applicants ONLY must please fill this application out; returning mentors will NOT fill this Form out, but will fill out a separate Form. This Form is for juniors who are not, but who wish to be a Mentor, as well as sophomores who were invited to apply.

This application will be open from Friday, March 31st through Monday, April 10th at 2pm.

Mentors must conduct themselves in an exemplary manner. How you behave academically and socially in and outside of school will, along with availability in the Program, be taken into consideration when determining your selection into the Mentor Program. If you are selected as a mentor, behavior that does not align with our core values is subject to dismissal from the Program.

Mentors will be selected by a committee of staff Advisors based on several factors, including (but not limited to) the application and the larger staff's own knowledge of the applicant. Please know that this Program traditionally receives several hundred applications, and while we wish we could accept every student, we are able to offer roles to very few; this is in no way a reflection of anyone's opinion of you, but rather of the extremely limited number of students that we are able to accept.

Thank you so much for applying for this Program! Decisions regarding candidacy will be sent to the email address that you list on this survey the week of April 24th.

IMPORTANT DATES:

Monday, April 10th at 2pm: Application is due; no applications will be accepted after this date/time.

Week of April 24th: Please check the email that you list on this application! Decisions regarding your candidacy will be sent to that address.
