While attending House of Dance LLC and any of its events or affiliated events (on or off site), there exists the possibility of injury, illness, or death. I accept and voluntarily incur all risks of any injuries, damages, or harm which arise during or as a result of my child’s participation in dance classes, regardless of whether or not caused in whole or in part by the negligence or other fault of House of Dance LLC or its employees. In the event you are unable to reach me, in case of accident or injury, I give permission for treatment as deemed necessary by staff, medical professionals, or emergency personnel.
I, the undersigned, agree not to hold House of Dance LLC, its owners, its affiliates, any of the chaperones, or any of its staff responsible in case of injury or accident and release them from all liability. I further agree to indemnify and hold harmless House of Dance LLC, and its agents and employees from claim, losses, injuries, and damages of any nature whatsoever incurred. House of Dance LLC dancers will be rehearsing and or performing at various locations including but not limited to Increasing Faith, auditoriums, arenas, and competition venues.
House of Dance LLC is not liable for any injuries or incidents that occur at Increasing Faith or other locations. By signing below I am confirming I have provided House of Dance LLC with a list of any medications, allergies, or previous injuries. I also certify that my child is healthy enough for dance activity.
