Code of Conduct 2023-2024

1.  LA HOSA Advisors will project a positive and professional image of Health Career  
     Education and HOSA to all those with whom they interact.

2.  LA HOSA Advisors will promote HOSA as a positive experience; therefore, will act  
     as a positive role model for students in dress, voice, attitude, actions and demeanor.

3.  LA HOSA Advisors are accountable to and for their students in all LA HOSA-related
     activities; Advisors will make every effort to be sure students attending LA HOSA  
     activities stay from beginning to end, and participate in all activities provided.

4.  LA HOSA Advisors are required to follow your district/school policy regarding transportation to/from HOSA  
     events as well as responsiblilty for and supervision of students while at HOSA events.   I will provide a
     copy of these school policies to LA HOSA each year.

5.  LA HOSA Advisors understand and follow established processes within the HOSA
     organization that protect the rights of all members.

6.  LA HOSA Advisors are expected to perform all assigned duties.

LA HOSA Advisors are proud of the standard of excellence they maintain for themselves and their students; attendance at any LA HOSA function implies acceptance and practice of these standards.

I have read the above Code of Conduct for LA HOSA Advisors and agree to accept and practice these standards.

Plan of Action: For failure to follow the Advisor’s Code of Ethics

1. Conference with the LA HOSA State Advisor and/or Board of Directors.
2. Written Report of the Plan of Action with a copy sent to local administrator of the  
    Advisor.
3. Dismissal from event or conference.
3. Recommendation for dismissal from HOSA by the LA HOSA Board of Directors.
4. Recommendation for dismissal from HOSA by the HOSA, Inc. Board of Directors.

I hereby grant the Louisiana HOSA, and National HOSA permission to make photographs, videotape, broadcasts, and/or sound recordings, separately or in combination, of me and permission to use the said photographs, videotape, broadcasts, and/or sound recording for educational and promotional purposes on any delivery system.

2023-2024 Statement of Assurance

This form indicates I will have in my possession for the duration of any activities, including travel to and from these activities a completed Photo Releae form, Code of Conduct, and Medical Liability Form for each student and participant including adults. This includes students attending any of the following Louisiana HOSA activities or any other workshops, seminars, and activities sponsored by Louisiana HOSA.

1. Fall Forum Leadership Conferences
2. State Leadership Conference
3. International Leadership Conference
4. Washington Leadership Academy
5. State Officer Training
6. Conference Planning Meetings for State Officers

1. Advisors should take the original copies of the students “Personal Liability and Medical Release Forms,” "Photo Release" and “Student Code of Conduct” with them to each event.

2. The Louisiana HOSA Comprehensive Consent Form, when properly and totally completed, represents my student’s and my best liability and medical protection during HOSA activities.
