AGOAC Programs are gradually re-opening.  The Chinese Health Dance CHD1 Program (Formerly called Yanji 元極舞(Intermediate), a non-instructional program, will resume for 3 months from January 2, 2023 to March 31, 2023 with limited capacity.  All participants in the Program are obliged to meet the Requirements by City of Markham , practice safe health protocol and maintain social distance.

1. This Form is used for 1 member only.  After you finish your form, submit a new form for another member.
2. Please note that after successful submission, you will get a receipt immediately : YOUR REGISTRATION FOR  CHD1 HAS BEEN RECORDED.  TO SUBMIT ANOTHER REGISTRATION, CLICK 'Submit another response' below. No other form of receipt will be issued.
3. Repeat Step 2 above for another member.
4. The program is pre-registered.  No drop in programs.
5. CHD1 Program is classified as Active with restriction and reduced limit capacity.  All Members must wear masks all time and anywhere in the Centre  except doing exercises.  During the break, you still have to wear the mask.
6.The Program runs from 10:00 am to 12:00 noon in Youth Centre with limited capacity.
