Please complete this form to ensure that the CVPS office receives documentation of a planned dismissal for your child. All forms MUST BE SUBMITTED prior to 1:30 p.m. on the day of the dismissal. There are no early dismissals after 3:00 pm. Thank you in advance for your cooperation.

Early Dismissal Pick Ups will occur at door 1, the main office. End of the Day Pick-Ups will occur at Door 5; the New Wing. Please remember to have your assigned placard with you.
