Please fill out the form below indicating you would like to have a booth at this year's conference.  Dues paying members of Suffolk County SFA may reserve ONE FREE BOOTH SPACE. You must complete and submit this form to reserve your space. We will provide one table and two folding chairs only for each booth. Please plan to furnish your own booth dividers.

Registration and payment are due no later than Friday, May 26th.  Space is very limited.  Spaces will be filled on a first-come, first-served basis.  Please note that you are responsible for payment of booth space and donation even if you do not attend.  Please contact (631) 382-4130 if you have any questions.
